Pricing As of August 1, 2021 our prices are as follows: New patient appointment $250 (in office)/$150 (telehealth) Follow up appointments $150 (in office)/$100 (telehealth)
We currently accept contracted insurances, cash, HSA/FSA payments, or credit card payments. Payment is due at beginning of the scheduled appointment. Should a payment be returned due to non-sufficient funds, a $20 processing charge will be incurred. As a courtesy to our patients, a superbill may be provided to you should you wish to submit a claim to your insurance for reimbursement according to your out-of-network provider network benefits. We are currently contracted with Hometown Health, Blue Cross Blue Shield, TriCare, Ambetter, Medicaid (panel currently full), & Medicare (panel currently full).
Cancellation Policy There is an automatic no-show appointment charge for appointments patients do not cancel with proper notice. We have a limited schedule that is booked months in advance with a long list of people wanting to be seen. If you are unable to make your appointment, please contact us 24 hours in advance so someone else can be helped.
