Appointments As of March 1, 2022 we no longer accept insurance. Our main focus is on Membership Medicine. It's a modern twist on the old fashion small town provider. Think of having your provider in your back pocket... cause you will with your phone! You choose! Take charge of your healthcare wants- membership or pay per visit offerings! We strongly believe in access to healthcare no matter your personal beliefs. Although we are focusing on our private Membership patients, we will be available one day a week for our non-member patients. Concierge Membership will be an annual enrollment fee of $49 per patient, plus monthly re-occurring fees . If you decide this plan may not be the best fit for you, please give 30 day notice of cancellation. Payment is due at beginning of the scheduled appointment. Should a payment be returned due to non-sufficient funds, a $20 processing charge will be incurred.
Cancellation Policy There is an automatic no-show appointment charge for appointments patients do not cancel with proper notice. We have a limited schedule that is booked months in advance with a long list of people wanting to be seen. If you are unable to make your appointment, please contact us 24 hours in advance so someone else can be helped.
